Do You See a Non-VA Health Care Provider?

VLER Health may benefit you! VLER Health is a program that shares certain parts of your Veteran's health record between the Department of Veterans Affairs(VA) and the Department of Defense(DoD) and selected private health care providers over a secure network known as the eHealth Exchange(eHE). This program is free and voluntary for the Veteran.

How might VLER Health benefit you?
Many Veterans receive a portion of their care from non-VA health care providers. Sharing certain parts of your electronic health record (EHR) will help reduce the need for patients and families to carry records between health care providers. It also will allow your private health care provider to make informed decisions about your care.

Why should you participate if you only get care at VA?
Unfortunately, accidents and unforeseen events happen. When they do, the VA Portland Medical Center might not be the closest hospital for emergency care. In an emergency, immediate access to additional health care information could save your life.

What parts of the record will be shared?
Health care providers will see a list of health problems, allergies, medications, vital signs, immunizations, and some laboratory test results. While we cannot share the entire health record, more information will be shared as the program grows, including doctor’s notes, discharge summaries, etc.

How do you join?
Signing up is fast and easy. You may sign up:

  1. in person
  2. by fax
  3. by mail
  4. online through MyHealtheVet portal
  5. online through eBenefits.

1. In Person: There are two locations to serve you. In Portland you can find us in the Portland VA Medical Center, 3710 SW US Veterans Hospital Road, Building 100, Room 1D-140. In Vancouver we’re located in the MyHealtheVet Program office, VAPORHCS Vancouver Division, 1601 E Fourth Plain Blvd, Building 15, Room 230, Vancouver, Washington. Ask for the  VA Authorization form (VA Form 10-0485) , complete it, sign it and hand it back to over to the MyHealtheVet Specialist and you’re done!

2. By Fax: Print, fill out, sign the VA Authorization form (VA Form 10-0485) and fax to: the MyHealtheVet Coordinator at (503) 721-7948.

3. By Mail: Print, fill out, sign the VA Authorization  form (VA Form 10-0485) and mail it to:

4. MyHealtheVet: A NEW easy way to sign online is by logging into your My HealtheVet account using DS Logon Premium (Username/Password).  If you don't have a My HealtheVet account, go to the My HealtheVet website and register today.

  1. Log on to:
  2. Select the My HealtheVet button
  3. Scroll down and select the Login with DS Logon button
  4. Review the terms and condition on the Secure Direct page and select Continue
  5. Enter your DS Logon Premium (Username/Password) and select Login
  6. You will be directed to your My HealtheVet account
  7. Select the Personal Information tab
  8. The Connect Your Docs box will display on the right.
  9. Select the select manage authorizations link
  10. Select Manage my Authorizations
  11. Select Start Authorization
  12. Review the terms and conditions and select Yes, to Continue
  13. Review the information on the screen and select Save and Continue
  14. Check the box and select Sign
  15. Re-enter your DS Logon Premium Username/Password and select Reauthenticate
  16. When the message in the green box appears you may View/Print your signed form
  17. Select the Close button at the top right to return to your My HealtheVet account

5. eBenefits: If you are a Premium* eBenefits account holder, we invite you to Connect Your Docs through the VLER Health Information Exchange.

Here’s how to sign up today to begin sharing your health records:

  1. To get started, go to and log in.
  2. Select Manage Health.
  3. Scroll to the Health Records row and select VA Health Record Sharing
  4. Select Manage My Authorizations and Preferences and then select Start Authorization.
  5. Accept the terms and conditions (you must click Yes to continue), then select Save and Continue
  6. Mark the box and select Sign. You will then be prompted to re-enter your username and password. 
  7. Finally, click Reauthenticate. (This next step may take a couple of minutes to process.)

Once you have completed one of these options, VA will be able to share your health data with community health care providers that are part of the VLER Health Program to better coordinate your care.

What if I decide not to join?
Participation is entirely voluntary. If you decide not to join VLER, we will not share your health information with private health care providers using the Health Exchange. This will not affect your benefits or your relationship with your Primary Care Provider at the VA Portland Health Care System.


What if I join and later change my mind?
If you change your mind, you can always complete a VA Revocation ("stop sharing") form (VA Form 10-0484). Or if it’s easier, you can always call us on our toll-free VLER Health Information line at 1-877-771-VLER (8537) to request one and we’ll mail it to you.
