Darwin G. Goodspeed was appointed as Director VA Portland Health Care System by the Secretary of the Department of Veterans Affairs on November 25, 2018. His previous position was Director of the Sioux Falls VA Health Care System (SFVAHCS) since December 2013. Prior to that he was the Associate Director at the Louis Stokes Cleveland VA Medical Center from February 2011 to November 2013. He also served as Acting Director of the Chalmers P. Wylie VA Ambulatory Care Center from May to June 2013. Mr. Goodspeed has more than 25 years of experience in health care leadership positions within VA, Department of Defense (DOD), and the private sector. He is a retired Naval officer having served 21 years on active duty first as a hospital corpsman and then as a Medical Service Corps officer. Mr. Goodspeed enlisted in the Navy in 1982 and was subsequently commissioned in 1990 and retired at the rank of lieutenant commander in 2003. Mr. Goodspeed holds a Master in Business Administration from Southern New Hampshire College, and a Master in Health Care Administration from Baylor University and an undergraduate degree in Business Management/Information Systems from the University of Maryland. He is a Fellow of the American College of Healthcare Executives (FACHE).

Ms. Karla Azcuy has served as the Deputy Director since November 2020. Karla began her VA career in 2009 at the VISN 20 Network Office as an Administrative Resident in the Graduate Healthcare Administration Training Program (GHAPT). After completing her training, she transitioned to VA Portland and has held multiple roles. Karla began her VA Portland career as a Staffing Human Resources Specialist until 2012. She spent more than five years with the Primary Care Division, serving as the Administrative Officer and then Administrative Director. In July 2017, Karla was appointed the Human Resources Officer. Karla previously served as the Acting Deputy Director in 2018 and late 2020. Karla received her Bachelor’s of Science degree from Clemson University in 2004, with a focus in Health Sciences. She received her Master’s in Health Administration from The University of North Florida in 2009. Karla completed the VISN 20 Leadership Development Institute (LDI) course in 2017, and was accepted into the 2021 Health Care Leadership Development Program (HCLDP). She is a member of the American College of Healthcare Executives (ACHE) and the Society for Human Resources Management (SHRM).

Bernard G. Deazley IV, was appointed as the Associate Director of VA Portland Health Care System in August 2019 and is responsible as the health care system Chief Experience Officer and the executive-in-charge of the Vancouver campus and 10 CBOCs. Prior to his appointment, Mr. Deazley served as the Assistant Director since 2014, Executive Assistant to Chief of Staff, and served as the VA Puget Sound Mental Health Outpatient Clinics operations manager. Mr. Deazley joined the VA through the student studies program following graduate school. During his VA tenure, Mr. Deazley has held positions of progressive responsibility in research and development, primary care, mental health, operative care and the executive staff. Mr. Deazley has also served as the Interim Director of the VA Southern Oregon Rehabilitation Center & Clinics (SORCC) from September 2020 to May 2021. Mr. Deazley is a graduate of the Annenberg Leadership Institute and VHA’s Healthcare Leadership Develop Program. Mr. Deazley received his Bachelor of Science degree from Willamette University (Salem, Oregon) and earned his Master of Public Administration degree at the Lewis & Clark College (Portland, Oregon).

Chief of Staff
Dr. Sahana Misra was appointed as the VA Portland Health Care System Chief of Staff (COS) in September 2018. Prior to her current appointment, Dr. Misra served as the Acting COS since September 2016; she was the VAPORHCS Mental Health Clinical Director from 2013 – 2016; the VAPORHCS Associate Chief of Psychiatry 2010 – 2013; and the Oregon Health & Science University (OHSU) Associate Psychiatry Residency Training Director 2001 – 2010. Following residency, she completed a Geriatric Psychiatry Fellowship at OHSU and began her VA career in 1998. Dr. Misra is board certified in Psychiatry and Geriatric Psychiatry. She was a VA Cooperative Studies Career Development Award recipient and has served as a principal investigator and co-investigator on several studies. Since joining VA Portland she has published 12 peer review articles, two review articles and three book chapters and has been an invited presenter at multiple national, regional and local lectures and conferences. Dr. Misra earned her MD at Rush Medical College and completed her Psychiatry Residency at Rush Presbyterian-St. Luke’s Medical Center in Chicago, Illinois. Following residency she completed a Geriatric Psychiatry Fellowship at OHSU and began her VA career in 1998 with VA Portland as a staff clinician. Dr. Misra is board certified in Psychiatry and Geriatric Psychiatry. In 2019, she was promoted to Professor in the OHSU Department of Psychiatry and was elected into the American College of Psychiatrists. Beyond VA Portland, Dr. Misra serves as OHSU’s Associate Dean of Veterans Affairs.

Deputy Chief of Staff
Dr. David Kagen has served as Interim Deputy Chief of Staff for VA Portland Health Care System since October 2016. After completing his internal medicine residency at Oregon Health & Science University (OHSU), Dr. Kagen began his career at VA Portland in 2004 as a staff hospitalist. He became Section Chief for Hospital Medicine in 2007, a position he held until his appointment in 2015 as Deputy Clinical Director for the Division of Hospital & Specialty Medicine (DHSM). Dr. Kagen received his Bachelor of Science degree from Duke University in 1996 and earned his MD from the University of Maryland in 2000. He holds an appointment as Associate Professor of Medicine at OHSU.

Deputy Director for Patient Care Services, Chief Nursing Officer
Dr. Clare O’Geary, DNP, was appointed as the VA Portland Health Care Deputy Director, Patient Care Services on June 6, 2021. Her previous position was the VA Portland Associate Chief Nurse Executive, Hospital and Specialty Medicine which she held since May 2018. Other VA experience includes Deputy Nurse Executive at the Charlie Norwood VA Medical Center in Augusta, GA (2011-2018); Chief Nurse Primary Care at the Oklahoma City VA Medical Center (2007-2011); Associate Chief Nursing Service, Education at the Oklahoma City VA Medical Center (1997-2007); and several Nurse Manager positions at the Oklahoma City VA Medical Center (1995-1997). Prior to joining VA, Dr. O’Geary worked 13 years in the private healthcare sector at California Pacific Medical Center in San Francisco and Fresno Community Hospital. She also was a member of the faculty at College of San Mateo and San Francisco State University. Dr. O’Geary earned her Associate Degree in Nursing from Fresno City College; her BSN and MSN from University of California, San Francisco; and her Doctor of Nursing Practice from Augusta University. She is certified at the Nurse Executive-Advanced level by the American Nurse Credentialing Center (ANCC) and as a Clinical Nurse Leader by the American Association of Colleges of Nursing (AACN). She is a Fellow of the American College of Healthcare Executives (FACHE).

Deputy Chief Nurse Executive
In August 2018, Dr. Jenny Richardson was selected as the Deputy Chief Nurse Executive (DCNE) for VA Portland Health Care System. Dr. Richardson joined VA Portland in 2006 as the Clinical Nurse Specialist (CNS) in Critical Care. In 2011, she transitioned into the CNS role in Primary Care to support implementation efforts surrounding the emerging PACT program. While in this position, she earned her Doctorate in Nursing Practice – Systems Leadership degree from Rush University (2014) with support from the national NNEI scholarship program for VA nurses. In 2014, Dr. Richardson assumed the acting and then permanent role as the Chief of the Education Division and Associate Chief Nurse Executive/Education. Upon retirement of the Associate Chief Nursing Officer/Nursing Professional Services in 2016, she also assumed oversight of that department. In 2017, Dr. Richardson graduated from the 2-year national VA Healthcare Leadership Development Program. Also in 2017, she was appointed to membership on the VAPORHCS Whole Health Flagship Executive Oversight Workgroup, and became Co-Chair of the Whole Health Committee. In March of 2018, Dr. Richardson was detailed into the Acting DCNE role until her most recent appointment.
