Pharmacy Residency Application Materials

The following information is required of all applicants to the Residency Programs at the Portland VA Medical Center:

1. Submit application material to PhORCAS- The Pharmacy Online Residency Centralized Application Service.  All material must be submitted electronically no later than January 11, 2013.


2. Please have three (3) letters of recommendation using the standardized form via PhORCAS. It is required for the references to complete the narrative comment section. Additional traditional letter is optional and may be uploaded along with the standard form.  Of the 3 references, one MUST be from a faculty member from your College of Pharmacy and one MUST be from a clinical pharmacy rotation preceptor who can discuss your work in direct patient care and/or hospital setting.


3. Additional application materials shall be required of successful applicants to the PVAMC Pharmacy Residency Program, including:

A. OF-306 Declaration for Federal Employment

B Application for Associated Health Occupations.
 

However, these will not be required of applicants during the initial application period. Instructions for completing these forms shall be provided to those selected for the residency.
