If you are a Nursing or Allied / Associated Health student you must complete the following course in preparation for your rotation at the Portland VA Medical Center. This course is mandatory, and required in order to meet VHA and Joint Commission hospital accreditation standards. Your completion will be tracked and recorded in a training record at the Portland VA Medical Center, and audits of these records will be conducted to ensure compliance.

IMPORTANT: You MUST be connected to a printer in order to print the required certificate at the end of the course. Your Certificate MUST be included with your application forms. NOTE; If you have problems with the course use the contact information on the course's website.

Complete / submit ALL  required forms below LEGIBLY AND IN INK!   And REMEMBER;

• You must submit the forms to the address below 4 weeks prior to your start date.
• Please be sure to sign and date wherever required.
• If you were not U.S. born, please include a photocopy of a citizenship document to show you are; legal to be in the U.S., i.e.: naturalization certification, resident alien card, passport, etc. The photocopy must be legible.
• Forms with the word “Optional” in the title are not optional – they are required!

Have the Program Director at your school complete and sign this Trainee Qualifications and Credentials Verification Letter (TQCVL);

The signed TQCV letter from the Program Director must be in a sealed envelope with his/her signature and date on the back of the envelope.

Then complete all of the following Education - Application Forms for WOC (WithOut Compensation) Students

• Cover Notes and Hybrid Form 0711
• VA Form 10-2850D Application for Health Professions Trainees
• Without Compensation Student / Instructor Registration Contract 
• Form SF 181, Ethnicity and Race Identification 
• VA Form 10-0410, Clinical Trainee Registration Form 
• Optional Form 306, Declaration for Federal Employment
